Step 1  - Download the high school team entry form (click here)

Step 2 - FAX the completed team entry form to 866-629-3833

Step 3 - Bring the signed forms with you to your first game, we will
collect them then.  All forms must be signed by all of your players.  
You may pay your entry fee below or bring a check/money order  
made out to "CHS Boys Basketball".  Cash will also be accepted and
we will give you a receipt.
